Personal Rights — Based on interviews and records review, the licensee did not comply with the section cited above by failing to provide a safe environment for children by allowing Child #1 to hit the cabinet until the bin fell on Child #1 in which poses an immediate health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on interviews and records review, the licensee did not comply with the section cited above by inadequately supervising Child # 1 while in the teachers’ care resulting in the child having access and drinking from a contaminated cup in which poses an immediate health, safety or personal rights risk to persons in care.

Personal Rights — Based on observations and interviews, the licensee did not comply with the section cited above by not having the hand soap and paper towels accessible to children in the children’s bathrooms and having gates that prevent exits in case of an emergency in which poses an immediate health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on observations and interviews, the licensee did not comply with the section cited above by failure to maintain play equipment, fixtures, and furniture in good repair, such as broken cargo net ladder, broken chairs, lumber, broken toys, shelves, and step stools and chipped paint on outdoor playground equipment, furniture, and dwelling) in which poses an immediate health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on observations and interviews, the licensee did not comply with the section cited above by leaving the children unattended in the bathroom while S1 was stepping away from the bathroom to go to the front door to open the door for LPA, and S2 was in the bathroom with S1 leaving the children unattended napping in Room 5 during nap time in which poses an immediate health, safety or personal rights risk to persons in care.

Responsibility for Providing Care and Supervision — Based on observations and interviews, the licensee did not comply with the section cited above by leaving the children unattended while the site supervisor was stepping away from the classroom to go to the front door to open the door for LPA, and the other teacher was in the kitchen preparing lunch for the children in which poses an immediate health, safety or personal rights risk to persons in care.

Teacher-Child Ratio — Based on observations and interviews, the licensee did not comply with the section cited above by not mainting the proper teacher-child ratios while the children were in the teachers' care in which poses an immediate health, safety or personal rights risk to persons in care.

Personal Rights — Based on interviews conducted, it was revealed that facility staff grabbed the childcare child’s arm, leaving a grab mark and causing pain. This poses an immediate health, safety, or personal rights risk to children in care.

Outdoor Activity Space — Based on observations and interviews, the licensee did not comply with the section cited above by failure to maintain outdoor activity space and failure to keep yards free from hazards in which poses a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — Based on observations and interviews, the licensee did not comply with the section cited above by failure to maintain the grounds under the play structures in which poses a potential health, safety or personal rights risk to persons in care.

Sign In and Sign Out — Based on the interviews and record reviews, it was proven that there were multiple occasions where center staff signed-in and/or signed-out the children which posed a potential health, safety, or personal rights risk to children in care.

Child's Records — Based on the interviews and record reviews, it was proven that child’s records were not given to child’s authorized representative, who had an authorization by law, which posed a potential health, safety, or personal rights risk to children in care.

Disaster and Mass Casualty Plan — Based on interview and record review, the licensee did not comply with the section cited above by not conducting the fire drill and the disaster drill at least once every six months in which poses/posed a potential health, safety or personal rights risk to persons in care.

Written directives for drinking water lead testing — The water report dated 06/18/2024 revealed that the facility had elevated levels of lead in one water faucet. This is a potential health and safety risk to the children in care.
