Teacher Aide Qualifications and Duties — Upon LPA's arrival and entering the facility, LPA's observed two (2)... .. assistant supervising four (4) preschool aged children without a qualifying teacher present which poses an potential risk to the health, safety or personal rights of children in care.

Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in which 5 of 5 children did not have physician's reportss on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — LPA's observed missing glass tiles on the classroom ceiling, large stains on classroom rugs, broken handle, chipped paint and spider webs on the outdoor play structure and feces camouflaged by the large wood chips. Exposed tarp under the chips poses tripping hazard. This is a potential risk to the health and safety of children in care.
Reporting Requirements — Facility failed to report to licensing an injury regarding day care child sustaining a fracture. This poses a potential risk to the health and safety of persons in care.
Child's Records — record review: licensee did not have an emergency information card for 7 out of 14 children. This poses a potential Health and Safety Risk to children in care.
