Infant Safe Sleep — Based on the record review, the Licensee did not meet the above regulation which poses a potential health and safety risk to the children in care. During the children’s file review, the LPA was unable to review an LIC 9227 Individual Infant Sleeping Plan for C4.
Family home — home free of safety hazards — Based on the record review, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During facility record review the LPA reviewed a fire drill log documenting the last drill was conducted on 8-20-2024. The Licensee understands fire drills must be conducted at least once every six months.
Mandated reporter training — Based on the staff record review, the Licensee did not meet the above regulation which poses a potential Safety risk to the children in care. During staff record review the LPA reviewed expired Mandated Reporter certificates for S1 and S1. Licensee understands all adults working with the children must have a current Mandated Reporter on file. Licensee understands Mandated Reporter training must be conducted every two years. Infant Safe Sleep — Based on the children record review, the Licensee did not meet the above regulation which poses a potential Safety & Health risk to the children in care. During children’s record review the LPA was unable to review sleep logs for C1. The Licensee stated she believed the sleep logs were for children up to one year old. Licensee understands sleep logs must be completed for children up to two years old.
Family home — preventive health training, including pediatric CPR and first aid — Based on the review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA reviewed a CPR/1st Aid for S1 and S2 that was not EMSA approved.
Staff immunization — influenza, pertussis and measles — Based on the review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During file review LPA reviewed immunizations for S1 which was missing MMR.
