This requirement was not met by: Licensees did not change diapers in a timely manner ressulting in diaper rashes, and/or did not properly clean excriment off children's private areas

Operation of FCCH- The home shall be free from defects or conditions which might endanger a child. Safety precautions shall include but not be limited to:...items which could pose a danger if readily available to children shall be stored where they are inaccessible to children. This was not met by: LPA observed a steak knife readily availble on the kitchen counter next to a cutting board and half lime, which poses and immediate health and safety risk to children in care.
Infant Safe Sleep — Based on LPA observation, the licensee did not comply with the section cited above when an infant was napping on a mat which poses an immediate health, safety or personal rights risk to persons in care.
Personnel Requirements — S1 being present in the FCCH on 08/04/2025 without a clearance or association to the license.

Mandated reporter training — Based on observation and record review, the licensee did not comply with the section cited above in 3 out of 4 personnel (Co-Licensee and 2 assistants) Mandated Reporter Certficates were unavailable, which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Records — Based on record review, the licensee did not comply with the section cited above in 2 out of 2 employee files were missing completed Notice of Employee Rights forms, which poses/posed a potential health, safety or personal rights risk to persons in care.
Family home — admission procedures and parent rights — Based on interview and record review, the licensee did not comply with the section cited above in 7 out of 8 childrens files reviewed were missing signed LIC 9224's (Acknowledgement of Receipt of Licensing Reports) for Type A deficiencies dated 01/20/2026, which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on interview and record review, the licensee did not comply with the section cited above in 2 out of 2 assistants did not have immunizations on file (1 assistant was missing Measles; 1 assistant did not have any immunizations on file), which poses/posed a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on record review, the licensee did not comply with the section cited above in that Licensee did not have a current roster, which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above an infant's file did not contain Sleep Logs for all his days in care which poses/posed a potential health, safety or personal rights risk to persons in care.
