Personal Rights — Based on LPA’s interviews, staff confirmed that an incident took place in May of 2022, where child #4 was injured while in care. Staff providing care and supervision disclosed being present however, statements from the staff who observed child #4 when child #4 was injured were inconsistent and neither staff present saw the incident occur. This poses a potential health and safety risk to children in care. Based on licensee's interview child #2 scratched child #1 during the time she was changing a diaper in the family room. The licensee did not observe the incident. This poses a potential risk to children in care
Reporting Requirements — Based on interviews conducted licensee did not notify parents of the injuries child #1 sustained until child was picked up at aproximately 5pm. This poses a potential risk to children in care.
