Current roster of children — Based on record review, it was determined that licensee did not have a roster available upon IB Investigator request. On 3/11/25, LPA reviewed the roster and the roster did not have the name's of the children present. This poses a potential risk to the health and safety to the children in care.
(b)(1) A report shall be made to the department by telephone or fax during the department's normal business hours before the close of the next working day following the occurrence during the operation of a family day care of any of the following events Based on record reviews it was determined that licensee became aware of the incident on 11/21/2024 and failed to report the incident within 24 hours to licensing office and did not submit the LIC 624B within 7 days. This poses a potential risk to the health and safety to the children in care.
Mandated reporter training — Based on record review, the licensee did not have current mandated reporter training certificate for LPA to review which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on record review, 7 out 7 children did not have a copy of their immunization records in their files which poses a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on record review, the licensee did not comply with the section cited above. During children file reviews, 4 out of 7 children present did not have a file which poses/posed a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on record review, the licensee did not comply with the section cited above in 5 out of 7 children did not have the LIC 627 form in their file which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above in the licensee could not provide the 15- minutes logs of the 1 infant that was present which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on record review, the licensee did not comply with the section cited above in the licensee's assistant did not have a copy of the immunization record in file which poses/posed a potential health, safety or personal rights risk to persons in care.
Child's Records — Based on observation, interview, and record review, the licensee did not comply with the section cited above. During children file reviews, LPA observed 2 out of 8 children present did not have a file which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation,and record review, the licensee did not comply with the section cited above. During children file review, LPA observed children in care were missing LIC 700 Identification and Emergency Information which poses/posed a potential health, safety or personal rights risk to persons in care.
