Responsibility for Providing Care and Supervision — Based on observation and interview, the Licensee left children alone in the preschool class without the supervision of a teacher which poses a potential health, safety, and/or personal rights risk to persons in care.
Limitations on Capacity and Ambulatory Status — based on observation, interview and documentation, staff has been providing care to infants and school age children which poses a potential health, safety or personal rights risk to persons in care.
Sign In and Sign Out — based on observation and records review, two children were removed from the facility without being signed out.
Fixtures, Furniture, Equipment and Supplies — based on observation, Licensee kept a playhouse with chipping paint and rust on the hand rails. The stairs to the playhouse were not sturdy. LPA also observed an 8 inch gap in the fencing that leads to an unlicensed area. The seat to the see-saw is loose and missing a bolt. The wooden poles in the outdoor area need to be sanded and painted.
Fixtures, Furniture, Equipment and Supplies — based on observation, the Licensee used the children's restroom to store supplies such as decorations, toys, and fire extinguishers.
Limitations on Capacity and Ambulatory Status — Based on observation and interview, the licensee did not comply with the section cited above. LPA observed Staff #1 (S1) providing care and supervision for a 8 month old infant which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care.
