The poisons were located in a cabinet that was not secured.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in 1 of 2 staff did not renew their Mandated Reporter training which poses a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in the licensee failed to provide proof of pediatric cardiopulmonary resuscitation and pediatric first aid pursuant to Health and Safety Code Section 1596.866 for a staff assigned to transport children daily offsite, to and from school for center activities, which poses a potential health, safety or personal rights risk to persons in care.
