(1) Commencing September 1, 2016, a person shall not be employed or volunteer at a day care center if he or she has not been immunized against influenza, pertussis, and measles. Each employee and volunteer shall receive an influenza vaccination between August 1 and December 1 of each year.
Buildings and Grounds — Based on observation, the director did not comply with the section cited above as director did not ensure poisons, including a Lysol spray bottle, Comet cleaner, and Clorox wipes, were locked which poses a potential safety risk to children in care. The poisons were located in a restroom cabinet with a magnetic latch near the diaper changing area.
Carbon monoxide detectors required — Based on observation and interview conducted, the director did not comply with the section cited above as director did not ensure a functioning carbon monoxide detector was present which poses a potential safety risk to children in care.
Infant Care Center Sleeping Equipment — Based on observation the licensee did not comply with the section cited above where LPA observed an infant child sleeping in the crib with a blanket and the licensee indicated the child has trouble sleeping without it, which poses a potential health, safety or personal rights risk to persons in care.
Infant Care Center Sleeping Equipment — Based on observation the licensee did not comply with the section cited above in LPA observed a material activity item attached to the inside of a crib, where a infant child was sleeping which poses a potential health, safety or personal rights risk to persons in care.
Infant Needs and Services Plan — Based on record review, the licensee did not comply with the section cited above in 2 infants under 12 months of age, present and in care did not have a LIC9227 Individual Sleep Plan and where the licensee stated they did not use the document, which poses a potential health, safety or personal rights risk to persons in care.
