Limitations on Capacity and Ambulatory Status — Based on record review, 9 children (C6-C14) were enrolled before their second birth date and 1 child (C5) was present at the facility today who has finished first grade which poses a potential risk to the health, safety, and personal rights of persons in care.

Staff were also recording children while on time out who were having a ‘fit’. This poses a health or safety risk to children in care.
