This requirement is not met as by: Based on interviews, observations, and review of pertinent documentation. The facility failed to provide a surface sufficient to cushion falls, which posed a potential health, safety or personal rights risk to persons in care. this violates the personal rights of children in care.
LACK OF SUPERVISION: 101229(a)(1) Care and Supervision. No child(ren) shall be left without the supervision, including visual observation, of a teacher at any time except as specified in sections 101216.2(e)(1) and 101230(c)(1). Based on the facility's self report, this requirement was not met as a child was left unsupervised. This poses an immediate health and safety risk to children in care.
Written directives for drinking water lead testing — Based on record review of the water sampling database, the facility was not in compliance with the above as of the initial water results uploaded on 9/23/2022. This posed a potential health, safety, or personal rights risk to persons in care.
