Drinking water lead testing — Based on record review, the facility did not comply with the section cited above by failing to conduct the lead test in a timely manner which posed a potential Health, Safety or Personal Rights risk to persons in care.
Food Service — Based on observation and confidential interviews, the facility did not comply with the section cited above by providing expired milk for daycare children which posed a potential Health, Safety or Personal Rights risk to persons in care.
Fixtures, Furniture, Equipment and Supplies — Based on observation, the facility did not comply with the section cited above when the facility provided play equipment with a broken window that has sharp edges which posed a potential Health, Safety or Personal Rights risk to persons in care.
