Personal Rights — Based on observation and interview, the facility did not comply with the section cited above as LPA at 12:20PM observed S1 remove C1 during scheduled nap time, the video footage was for an incident that occured Febuary 7, 2025 which poses a potential health, safety or personal rights risk to persons in care.
