Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in Staff (S1) was missing proof of pertussis and S2) was missing proof of pertussis and measles vaccinations which poses a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — During facility walk through LPAs observed that the sandbox has a lot of debris and is not covered when not in use.
Buildings and Grounds (a)The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. During facility walk through, LPAs observed the side outdoor area has discarded pipes, gardening tools, broken children's table, paint dispenser cans, raid fly trap sticks and hose storage cans filled with water and debris in the area on limit to children in care. — requirement not met
Drinking water lead testing — Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During interview, staff stated that water bottles are refilled using store bought gallons of water but kitchen sink is used to wash fruit, vegetables and children's dishes and utensils. Water lead testing has not been completed for this facility.
Admission Policies — Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA did not observe Notification of Parents' Rights (PUB 393), Child Passenger Restraint System Poster (PUB 269) and Personal Rights (LIC 613A) posted near the front entrance.
Child Care Center Director Qualifications and Duties — Based on observation and interview, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. LPA did not observe facility director during today's visit as she is unwell. During staff interview, staff stated that facility director is present once a week at the facility.
Personnel Records — Based on observation, interview and record review, the licensee did not comply with the section cited above for two staff , S1 and S2 which poses a potential health, safety or personal rights risk to persons in care.
Child's Medical Assessments — Based on observation, interview and record review, the licensee did not comply with the section cited above for three children, C1, C5 and C6 which poses a potential health, safety or personal rights risk to persons in care.
Immunizations — Based on observation, interview and record review, the licensee did not comply with the section cited above for two children, C5 and C6 which poses a potential health, safety or personal rights risk to persons in care.
