102417 Operation of a Family Child Care Home (a) When circumstances require the licensee to be temporarily absent from the home, the licensee shall arrange for a substitute adult to care for the children during their absence. Temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day. Based on interview with licensees, Ms. Amiri was out of the country for an extended period and was not present 80% of the time the facility was operating. This poses a potential risk to the health and safety of children in care.

Immunizations — Based on observation and interview, the licensee did not comply with the section cited above as 7 of 25 children enrolled did have immunization records in their files. This poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on interview and record review, the licensee did not comply with the section cited above in that 3 of 3 three infants in care did not have Safe Sleep Logs on file. This poses a potential health, safety or personal rights risk to persons in care.
