Alterations to Existing Buildings or New Facilities — Based on observation, interview and file review, the licensee did not comply with the section cited above. The facility is licensed for room 9 only, based on today's visit, preschool is conducting in room 51. During interview with Director, Director stated the preschool moved to room 51 back in 2020 and the play yard is located in the same area but has been remodeled. This poses/posed a potential health, safety or personal rights risk to persons in care.
