Responsibility for Providing Care and Supervision — A 5 year old wandered away from the facility and was found crossing back and forth across the street by 2 motorist. The motorist returned the child back to the facility.

Responsibility for Providing Care and Supervision — Based on observation, the licensee did not comply with the section cited above when children were left unsupervised which poses an immediate risk to the health, safety or personal rights of children in care.

Buildings and Grounds — Based on observation and interviews, the licensee did not comply with the section cited above when multiple cleaning supplies were accessible to children which poses an immediate risk to the health, safety or personal rights of children in care.

Food Service — Based on observation, the licensee did not comply with the section cited above facility failure to provide enough snacks for children which poses an immediate/potential risk to the health, safety or personal rights of children in care.

Teacher-Child Ratio — Based on observations, the licensee did not comply with the section cited above when LPAs observed 1 fully qualified teacher supervising 13 children alone for 2 hours which poses an immediate risk to the health, safety or personal rights of children in care.

Responsibility for Providing Care and Supervision — LPAs observations of children within facility without visual supervision.

Buildings and Grounds — LPA observations of flies in facility, cleaning products accessible to children, trash and the lack of sanitary practices throughout the facility

Responsibility for Providing Care and Supervision — Daycare children were not adequately supervised during an incident where the children climbed a fence and threw rocks next door. LPA's also observed children not being visually supervised on 8/13/205 which poses a potential risk to the Health, Safety or Personal Rights to children in care.

Personal Rights — Based on conducted interviews, it was determined that staff yells at children in care which poses a potential risk to the Health, Safety or Personal Rights to children in care.

Written directives for drinking water lead testing — the lead testing results received, indicating that two outlets in the facility had results in exceedance of the 5 ppb Action Level. This poses a potential risk to the children in care.
