Personal Rights — Based on interviews, the licensee did not ensure that each child is accorded their personal rights. C4, C6, and C7 disclosed S1 yells at daycare children. S1 disclosed that she/he raise the voice to children. This poses an immediate risk to the personal rights of children in care.

Responsibility for Providing Care and Supervision — Based on records reviewed, the licensee did not ensure that one child was visually supervised while in care. Records show the licensee admitted that a child ran into the parking lot alone while in care.

Responsibility for Providing Care and Supervision — Based on observation and interview statements, the requirement was not met for one of ten children, which poses an immediate risk to the health, safety, and personal rights of children. C1 was left without supervision for around 5-10 minutes.

Personnel Records — Based on interview statements, the director did not ensure documentation of the hours actually worked by facility personnel. The director stated that hours worked are not kept for S1, S3, and S4.

Drinking water lead testing — Based on interviews, the licensee did not comply with the section cited above in one of one water lead tests, which poses/posed a potential health, safety or personal rights risk to persons in care. The didrecto stated the facility did not test the water for lead. The director uses a Coway system that filters tap water. This source of water has not been tested.

Mandated reporter training — Based on records review and observations, the licensee did not comply with the section cited above in three out of three staff, which poses/posed a potential health, safety or personal rights risk to persons in care. Three staff, two of whom were present, did not have a valid mandated reporter certificate.
