(e) If no assistant provider is present at a Large Family Child Care Home, then the licensee shall comply with the capacity requirements for a Small Family Child Care Home as specified in subsections (b) and (c). This was not met as evidenced by: Although the home is licensed as a Large Family Child Care Home, the licensee did not have a qualified assistant present, but had 9 chidlren present. this poses/posed an immediate risk to the health and safety of children in care.

(i) If an infant falls asleep before being placed in a crib or play yard, the provider shall move the infant to a crib or play yard as soon as possible. This was not met as evidenced by: LPA observed 2 sleeping infants in swings. This poses/posed an immediate risk to the health and safety of children in care.

Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above in that, upon entry the barricade that prevented access to the stairs was not engaged and there were children present that were under the age of 5. This poses/posed a potential health, safety or personal rights risk to persons in care.

Family home — home free of safety hazards — Based on observation and interview, the licensee did not comply with the section cited above in there were 6 total items that stated "call poison control" on their label. These items were inaccessible to children in a latched cabinet underneath the kitchen sink. However, these were not secured by a key lock or combination lock. This poses/posed a potential health, safety or personal rights risk to persons in care.

(a) The licensee shall be present in the home& ensure children in care are supervised at all times. When circumstances require licensee to be temporarily absent from the home, the licensee shall arrange for a substitute adult to care for and supervise the children during his/her absence. Temporary absences shall not exceed 20 percent of the hours that the facility is providing care per day. This was not met as evidenced by: video footage showing licensee leaving the property during the time children were in care. This poses a potential risk to the health and safety of children.

Family home — home free of safety hazards — Based on licensees own admission, the above regulation was not in compliance. Licensee has not conducted any disaster drills with the daycare children. This poses/posed a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on record review and licensee own admission, the above regulation was not in compliance. 1 out of 3 children did not have immunizations on file. Licensee stated that parents do not wish to get the child immunized. This poses/posed a potential health, safety or personal rights risk to persons in care.

Family home — admission procedures and parent rights — Based on record review, the licensee did not comply with the section cited above in 1 out of 3 children. C3 did not have a signed LIC995 on file. This poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on licensees own admission during record review, the licensee did not comply with the section cited above. Licensee has not been documenting 15 minute sleep checks. This poses/posed a potential health, safety or personal rights risk to persons in care.
