If no assistant provider is present at a Large Family Child Care Home, then the licensee shall comply with the capacity requirements for a Small Family Child Care Home as specified in subsections (b) and (c). Based on observation and interview, the licensee did not comply with the section cited. On 2/20/26, Licensee’s assistant Katherine Payan was alone with nine children and one infant. This is an immediate health, safety, and personal rights risk to children in care.
Family home — preventive health training, including pediatric CPR and first aid — Based on observation, interview, record review, the licensee did not comply with the section cited above. Licensee assistant was alone with children on 2/20/26, without a current EMSA approved CPR & 1st Aid training, which posed an immediate risk health, safety or personal rights risk to persons in care.
(a) Each child receiving services from a family child care home shall have certain rights that shall not be waived…(2) To receive safe, healthful, and comfortable accommodations, furnishings, and equipment. Based on witness disclosure in the (DCFS) Investigation Report signed 2/20/26 and LPA witness interviews; (between 12/2025 & 02/2026), the Licensee & resident assistant drank alcohol during daycare hours & the Licensee operated the daycare under the influence of alcohol. This poses/posed a immediate risk health, safety or personal rights risk to persons in care.
Criminal Record Clearance — Based on interviews with witnesses and licensee, the licensee allowed an unqualifed and uncleared person on December 4, 2023 to work at the daycare, the licensee did not comply with the section cited above which poses/posed an immediate health, safety or personal rights risk to persons in care.

On 2/20/26, children were sleeping in unsafe uncomfortable sleeping arrangement causing two preschoolers to block emergency exits, which posed a potential health, safety or personal right to persons in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above. Assistant does not have proof of a current Mandated Reporter training certificate on 2/20/26, which posed a immediate risk health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — based on interview and record review, the licensee did not comply with the section cited above. Assistant did not have immunizations record while caring for children on 2/20/26, which posed a potential health, safety or personal right to persons in care.
Infant Safe Sleep — Based on observation, the licensee did not comply with the section cited above in 1 out of 2 infants present. LPA's Child 2 head covered with a blanket upon entering the room which poses a potential safety risk to persons in care.
