(n) Furniture and equipment shall be maintained in good condition, free of sharp, loose or pointed parts. This was not met as evidenced by: During a visit to the facility on 07/23/2025, LPA observed 3 cracks and pieces missing from a bench in the outdoor play area. LPA sat at the table during children's outdoor playtime today and saw the same exact cracks still there. Children were seen playing around the cracked bench. This was addressed under a Technical Violation (TV), but is now being cited as a Type A citation. This poses an immediate health and safety risk to the children in care.

This was not met as evidenced by: During a tour of the facility, although most classrooms had chemicals stored behind a key lock, Room 4 and the restroom between rooms 1&2 had cleaning supplies that were poisonous and not behind a key lock. Preschool Program with Toddler Component — (a) Licensees serving preschool-age children may create a special program component for children who are between 18 months and 36 months of age. The provisions of Sections 101151 through 101239.2 shall apply for children over 24 months, in addition to those specified in Health and Safety Code section 1596.955. The provisions of Sections 101351 through 101439.1 shall apply for children between the ages of 18 and 24 months participating in a preschool toddler component, in addition to those specified in Health and Safety Code section 1596.955. (1)A child who is between 18 months and 36 months of age may participate in the toddler program with written permission from the child's authorized representative. No child in the toddler program shall be placed in the preschool program before the age of 30 months without written permission from the child's authorized representative. Based on record review and staff's own admission, the licensee did not comply with the section cited above. There is a form that states: "I consent for my child to be commingled with older preschool students during the first and last hour of the toddler program as needed (7:00-8:00am & 4:00-5:00pm)." Although toddlers 24 months of age may be commingled with parent consent, toddlers under 24 months cannot. This poses/posed a potential health, safety/ personal rights risk to persons in care.
