Criminal background check — Based on observation, interview and record review, the licensee did not comply with the section cited above for staff, S1 which poses a potential health, safety or personal rights risk to persons in care. One staff, S1 was observed to be cleared but not associated to the facility. Director reviewed Guardian and observed a request for additional verification. Director completed the association during today's inspection. LPA reminded Director to verify staff association's prior to staff being present or working at the facility.
Responsibility for Providing Care and Supervision for Infants — Based on interview, the licensee did not comply with the section cited above for children under 2 years of age that are enrolled in the toddler componenet which poses a potential health, safety or personal rights risk to persons in care.
Health-Related Services — Based on observation, interview and recrod review, the licensee did not comply with the section cited above for one child, C11 which poses a potential health, safety or personal rights risk to persons in care.
Infant Care Teacher Qualifications and Duties — Through file review LPA observed that staff S3 is missing transcripts that indicate proof of completion of early childhood education units to be a fully qualified teacher. This poses a potential risk to the health, safety and personal rights of children in care.
Personnel Records — Through file review LPA observed that staff, S2, S3, S4 and S5 are missing the required Personnel Record (LIC 501). This poses a potential risk to the health, safety and personal rights of children in care.
Mandated reporter training — Through file review LPA observed that staff, S1 is missing the Mandated Reporter Training for childcare workers. This poses a potential risk to the health, safety and personal rights of children in care.
Staff immunization requirements — Through file review LPA observed that staff S1, S3 and S5 are missing proof of immunization's for measles and pertussis. This poses a potential risk to the health, safety and personal rights of children in care.
