Napping Equipment — Based on observation and interview, the licensee did not comply with the section cited above. Children's blanket/sheets were observed stored in cabinets and observed beddings coming into contact with other beddings. This poses a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above. LPA observed several holes on the playground's surface in the children's play area. This poses a tripping hazard. The shade/sails in the play area was observed too low on the ground. This poses a potential health, safety or personal rights risk to persons in care.
Drinking water lead testing — Based on interview the licensee did not comply with the section cited above. Facility failed to obtain Water Lead Testing as their building was built prior to January 2010. This poses a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on interview and record review, the licensee did not comply with the section cited above. Several staff did not have proof of being immunizaed against influenza, measles & pertussis. This poses a potential health, safety or personal rights risk to persons in care.
