Staffing Ratio and Capacity — On 06/30/26, LPA observed 23 children were present in the home with licensee's four assistants, licensee was operating out of her license limitation with additional 8 children. This poses an immediate health, safety and personal right risk to the children in care.

Family home — children supervised at all times — On 06/30/26, licensee was not present in the home, who called and informed LPA that she went to Armenia on 06/25/26 due to family emergency and facility will be open during her absence and did not nofity the department. This poses a potential health, safety and personal right risk to the children in care.
Family home — home free of safety hazards — During this inspection, facility representative, Anahit Zakharyan was not able to provide a copy of facility roster to LPA. This poses a potential health, safety and personal right risk to the children in care.

Roster did not include new enrolled children`s name and did not have date out for children that left, which poses/posed a potential health, safety or personal rights risk to persons in care.
Operation of a Family Child Care Home — Based on observations and records review ,the licensee did not comply with the section cited above. LPA reviewed 14 children files and 4 children did not have formed signed , which poses/posed a potential health, safety or personal rights risk to persons in care.
