When alone and unsupervised inappropriate contact occurred between C1 and C2. This poses an immediate risk to the health, safety or personal rights of the persons in care.
Indoor Activity Space for School-Age Children — Based on record review and interviews, C1 a school age child was not separate from the preschool children on 10/28/24. C1 and C2 engaged in inappropriate contact in the preschool classroom, this posed an immediate risk to the health and safety of persons in care.
Responsibility for Providing Care and Supervision — Based on observation, the licensee did not ensure C1 was not left without supervision, as C1 was observed alone in the toddler room at 1:45pm and 2:40pm which poses an immediate health and safety risk to children in care. Civil penalty of $500 was assessed.

Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above in when Lysol, disinfectant wipes and fabreeze were observed in accessible drawer within the staff restroom, which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Records — Based on interview and record review, the licensee did not comply with the section cited above when one of one volunteers did not have a personnel record containing the required information, which posed a potential health, safety or personal rights risk to persons in care.
