There was one staff member providing care and supervision of children that do not have a criminal record clearance or an exemption from the State Department of Social Services prior to employment, residence, or initial presence in the facility which poses an immediate health, safety or personal rights risk to persons in care.

Drinking water lead testing — Based on interview and record review the licensee did not comply with the section cited above. This facility is unable to provide evidence of completed water testing. which poses/posed a potential health, safety or personal rights risk to persons in care.
Disaster and Mass Casualty Plan — Based on interview and record review, the facility did not comply with the section cited above by not providing proof of conducting disaster and fire drills as requiered which poses/posed a potential health, safety or personal rights risk to persons in care.
Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above when LPA observed two broken windows in the front of the center. which poses/posed a potential health, safety or personal rights risk to persons in care.
Fixtures, Furniture, Equipment and Supplies — Based on observation the licensee did not comply with the section cited above when LPA observed 6 children on an outdoor play structure that was not properly anchored to the ground posing a potential safety risk to children in care if it tipped over due to the distribution of weight from the children. which poses/posed a potential health, safety or personal rights risk to persons in care.
