Criminal Record Clearance — Based on analyst observation and interview with licensee, the licensee did not comply with the section cited above as acquaintance, Larissa Kabaye, was found actively engaging in the facility today without fingerprint clearance which is an immediate risk to the health and safety of children in care.

Based on LPA interviews conducted, and records reviewed, the licensee did not comply with the above regulation in that the licensee did not ensure that the licensee’s adult daughter’s criminal record clearance was transferred & associated to the day care facility license, which poses an immediate Health & Safety risk to children in care.
Staff immunization — influenza, pertussis and measles — This is an amended version of the original report created on 04/3/2025. Based on LPA interviews conducted, and records reviewed, the licensee did not comply with the above regulation, the licensee did not maintain documentation of the required immunizations records for their adult son, which poses a potential Health and Safety risk to children in care.
