Personal Rights — Based on interview and video footage, S1 handled C1 in a rough manner and did not ensure the personal rights of C1, therefore the licensee did not comply with the section cited above which posed a possible health and safety risk to persons in care.
Outdoor Activity Space — Based on observation,, the licensee did not comply with the section cited above where during the walk through, LPA observed a large ladder in the childerns outdoor activity space, DIR removed during inspection, which poses/posed a potential health, safety or personal rights risk to persons in care.
Child's Medical Assessments — Based on interview and record review, the licensee did not comply with the section cited above where during file review, C4 file was missing a complete written medical assessment which poses/posed a potential health, safety or personal rights risk to persons in care.
Admission Policies — ; Based on interviews conducted and evidence gathered, on October 24, 2025 the CCC allowed a pick up that conflicted with a court order, which poses/posed a potential health, safety or personal rights risk to persons in care.
101212(d) Reporting Requirements Upon the occurrence, during the operation of the childcare center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days following the occurrence of such event. This requirment was not met as evidenced by; Based on interview and record review, the CCC did not meet this regualtion in 3/3 recent unusual incidents occuring at the CCC within the last 12 months were not reported to CCLD.
Notice to parents that site visit reports are available — ; Based on interview and record review, the licensee did not comply with the section cited above where the CCC could not provide evidence that newly enrolled families/children have been provided copies of any licensing document that the licensee has received during the prior 12-month period resulting in a Type A deficiency, which poses/posed a potential health, safety or personal rights risk to persons in care.
Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in 1 out of 6 persons were missing their TB clearance which poses/posed a potential health, safety or personal rights risk to persons in care.
