Infant Care Teacher Qualifications and Duties — review of staff transcripts and staff interviews that staff do not have infant/toddler class which posses an immediate health & safety risk to the children in care.

Fire Clearance — toddlers in the area where a fire clearance was granted for children ages 6-12 on 3/28/2024 which posses an immediate health & safety risk to children in care.

Staff-Infant Ratio — Based on record review, the licensee did not comply with the section cited above in that there was no fully qualified infant teacher for 2 infants which poses an immediate health, safety or personal rights risk to persons in care.

Disaster and Mass Casualty Plan — Based on record review, the licensee did not comply with the section cited above in that there was no documentation of conducted fire drill logs which poses a potential health, safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based on observation, the licensee did not comply with the section cited above in that there was a garbage can in the toddler room that was used for lunch leftovers with no lid which poses a potential health, safety or personal rights risk to persons in care.

Infant Care Center Fixtures, Furniture, Equipment, and Supplies — Based on observation, the licensee did not comply with the section cited above in that the changing table in the infant napping room has a portable sink that does not have water for staff to wash hands which poses a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in that 2 out of 3 staff did not have immunization record which poses a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in 3 out of 3 staff did not have record of health-screening report which poses a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in that 5 out of 5 children did not have physician's report (LIC 701) which poses a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on record review, the licensee did not comply with the section cited above in that 4 out of 5 children did not have proof of immunization which poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Care Food Service — Based on observation, the licensee did not comply with the section cited above in that no bottles were labeled with infant names or dated which poses a potential health, safety or personal rights risk to persons in care.
