In reviewing children files, LPA determined that the licensee had not documented the 15 minute sleep checks since February 2025, for child/infant(s) #1, #2, and #3. In reviewing children files, Child #1, #2, #3, #5, & #6, were missing an Acknowledgement of Receipt of Licensing Reports (LIC9224) for a Type A citation that was issued on 8/15/2024.
