Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above. In touring the bathroom that is assigned to the children in care LPA observed soap, lotion, hair products, and mouth wash. These items have a warning to, Keep out of the reach of Children, posted on the label. The cabinet was accessible due to the locking mechanism not being placed/installed correctly. This poses an immediate health, safety or personal rights risk to persons in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above. In reviewing staff files, LPA was not able to locate proof of completion for the required training. This poses/posed a potential health, safety or personal rights risk to persons in care.

Infant Safe Sleep — Based on record review, the licensee did not comply with the section cited above. In reviewing children files, LPA determined that the licensee had not documented the 15 minute sleep checks since February 2025, for child/infant(s) #1, #2, and #3. This poses/posed a potential health, safety or personal rights risk to persons in care.

Notice to parents that site visit reports are available — Based on record review, the licensee did not comply with the section cited above. In reviewing children files, Child #1, #2, #3, #5, & #6, were missing an Acknowledgement of Receipt of Licensing Reports (LIC9224) for a Type A citation that was issued on 8/15/2024. This poses/posed a potential health, safety or personal rights risk to persons in care.

Child's Records — Based on record review, the licensee did not comply with the section cited above. In reviewing children records, LPA noted that Child #2 did not have the required LIC 995A form on file for review. This poses/posed a potential health, safety or personal rights risk to persons in care.
