Large family home licensing requirements — Based on the observation, the Licensee did not meet the above regulation which poses an immediate health & safety risk to the children in care. During the facility tour, the Licensee tested three smoke detectors all of which were not in working condition.

During the facility tour the LPA observed a carbon monoxide and smoke detector not inworking condition. Licensee stated both detectors need batteries.

Large family home licensing requirements — Based on the observation, the Licensee did not meet the above regulation which poses an immediate Health, Safety & Personal Rights risk to the children in care. During the facility tour LPA observed the only carbon monoxide detector without a battery.

Immunizations — Based on the record review, the Licensee did not meet the above regulation which poses a potential health risk to the children in care. During the children’s file review, the LPA was unable to review immunizations for C4.

Family home — home free of safety hazards — Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. Licensee stated she could not locate the fire drill.

Infant Safe Sleep — Based on the record review, the Licensee did not meet the above regulation which poses a potential safety to the children in care. During the children’s file review the LPA was able to review sleep logs, however, the sleep logs were documented every 20, 30, 45 minutes.

Inspection Authority of the Department — Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During the children’s file review the LPA was unable to review a file for C6.

Immunizations — Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During children’s file review the LPA was unable to review immunizations for C2, C3, C5 and C6.

Family home — admission procedures and parent rights — Based on the record review, the Licensee did not meet the above regulation which poses a potential Personal Rights risk to the children in care. During the children’s file review the LPA was unable to review and LIC 995 for C7.

Family home — home free of safety hazards — Based on the record review, the Licensee did not meet the above regulation which poses a potential safety risk to the children in care. During facility records review the LPA was unable to review a current roster.

Mandated reporter training — Based on the interview and record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During record review LPA reviewed expired Mandated Reporter certificates for the Licensee and S2. Licensee stated there is no current certificate available and training has not been taken.

Child's Records — Based on the interview, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During interview Licensee a file is not available for C5.

Family home — home free of safety hazards — Based on the record review, the Licensee did not meet the above regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During record review LPA reviewed a facility roster that did not include C4 and C5.

During record review LPA was unable to review sleep logs for C3. Licensee stated she was not aware the checks had to be documented.
