Based on guardian search S1 did not have background clearance. This poses an immediate health and/or safety risk to children in care.

Based on roster review, guardian background check search and interviews conducted. S1 and S4 have been present and working at the facility without background clearance. This poses an immediate health and safety risk to children in care.

Teacher-Child Ratio — Based on observation and interviews, one teacher and one volunteer were supervsing 17 preschool children at one time which poses a potential risk to the health, safety, and personal rights of persons in care.

Criminal Record Clearance — Based on interview and record review, one new staff worked at the facility on 06/08/26 without an associcated criminal record clearance, which poses an immediate health & safety and personal rights risk to the children in care.

Personnel Records — Based on record review, one volunteer did not have a health statement on file which poses a potential risk to the health, safety, and personal rights of persons in care.

Personnel Records — Based on record review, one volunteer did not have a Tuberculosis Test on file which poses a potential risk to the health, safety, and personal rights of persons in care.

Limitations on Capacity and Ambulatory Status — Based on observation and interviews, one infant was observed on the preschool license which poses a potential risk to the health, safety, and personal rights of persons in care.

Teacher-Child Ratio — Based on observation and interviews, on multiple occasions, unqualified staff was supervising over 12 day care children at one time which poses a potential risk to the health, safety, and personal rights of persons in care.

Activities/Napping — Based on LPA observations, facility was out of ratio as 3 of 17 children were awake while napping and being supervised by two unqualfied staff and no teacher which poses a potential risk to the health, safety, and personal rights of persons in care.

Reporting Requirements — Based on D1 admitting to not reporting the outbreak that began on 04/07/26 and the Department has no record of the incident reported to the Department within the Department’s next working day during normal business hours. This poses a potential health, safety or personal rights risk to persons in care.

Criminal Record Clearance — Based on observation, record review, and interview D1 did not associate 1 of 2 staff present today criminal record clearances to the facility. S2’s criminal record clearances was not transferred to the facility prior to the first day of employment, which poses a potential health and safety risk to persons in care.

No child(ren) shall be left without the supervision of a teacher at any time, except as specified in Sections 101216.2(e)(1) and 101230(c)(1). Supervision shall include visual observation. The requirement was not met as evidenced by: Based on staff interviews, a child (C1) was allowed to use the bathroom on 12/17/25 by themselves without staff supervision which poses a potential risk to the health, safety, and personal rights of children in care.

Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in which C1-C5's did not have completed physican reports which poses a potential health safety or personal rights risk to persons in care.

Immunizations — Based on record review, the licensee did not comply with the section cited above in which 2 out of 5 children's records (C1 & C2) did not have immunization records as required which poses/posed a potential health, safety or personal rights risk to persons in care.

Teacher-Child Ratio (a) There shall be a ratio of one teacher visually observing and supervising no more than 12 children in attendance. . . This was net met as evidence by... based on interviews on at least one occasion there was one staff left alone supervising more than 12 children this poses a health and safety risk to children in care. — requirement not met

Teacher Qualifications and Duties Prior to employment, a teacher shall meet the requirements of (b)(1) or (b)(2)… (b)(1) A teacher shall have completed, with passing grades, at least six postsecondary semester… or (b)(2) A teacher shall meet the requirements of Health and Safety Code Section 1597.055. This was not met as evidence by... Based on interviews and observations made, On at least two occasions Aides (Staff with 0 early child hood education units) were working in the capacity of a teacher supervising children alone. This poses a potential health and safety risk to children in care.

101212 Reporting Requirements. Upon the occurrence, during the operation of the child care center of any of the events specified in (d)(1) below, a report shall be made to the Department by telephone or fax within the Department's next working day and during its normal business hours. In addition, a written report containing the information specified in (d)(2) below shall be submitted to the Department within seven days . . . Thi was not met as evidence by. . . Based on evidence received and interviews conducted staff did not report an incident resulting in an injury requiring medical treatment. 101215.1 Child Care Center Directors Qualifications and Duties. If the child care center director is absent for more than 30 consecutive calendar days, the substitute director shall meet the qualifications of a director. This was not met as evidence by. . . based on the interviews conducted and records reviewed there was not person on the site that is a qualified director. This poses a potential health and safety risk to people in care.
