Personal Rights — Based on interviews, the licensee did not comply with the section cited above in 1 out of 1 interactions where staff yelled at a child, which This poses/posed a immediate Health and Safety risk to children in care.

Limitation on Capacity. A licensee shall not operate a childcare center beyond the conditons and limitations specified on the license, including the capacity limitation. The requirement is not met as evidence by: Based on interviews conducted with previous director, staff, and children, the school age and preschool age children are commingling in the early morning and late afternoon. This poses an immediate health and safety risk to children in care.

Buildings and Grounds. The childcare center shall be clean, safe, sanitary, and in good repair at all times to ensure the safety and well-being of children, employees, and visitors. This requirements is not met as evidence by LPAs observation of a broken cabinet containing cleaning supplies accessible to children in care. This poses an immediate health and safety risk to children in care.

Responsibility for Providing Care and Supervision — LPAs observation of two school aged chidren at two different times left the outdoor play area and came into the facility alone without a staff members knoowledge. LPAs had to inform the staff member that children came inside alone. This poses an immediate risk of childrens health and safety in care.

Criminal Record Clearance — LPA observed during file review and staff names taken, staff #4 not fingerprint cleared and staff #1 is not fingerprint associated to this facility. This poses an immediate risk to the health, safety, and personal rights of child in care. A civil penalty is being assessed.

Drinking water lead testing — Based on interviews, the licensee did not comply with the section cited above. The facility has not had their water lead testing done. This poses an immediate risk to the health, safety, and personal rights of child in care

Indoor Activity Space for School-Age Children — Per interviews and documents reviewed, it was stated that preschool and school age children are commingled in the morning and in the afternoon due to staffing issues. This poses an immediate risk to the health, safety, and personal rights of child in care.

Teacher Aide Qualifications and Duties — An aid and van driver, and who opens the day care has been left alone with Preschool and school age children and no other staff around in the morning and in the afternoon. This poses an immediate risk to the health, safety, and personal rights of child in care.

Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. LPAS OBSERVED THE CARPET IN THE MAIN SCHOOL AGE CLASS ROOM IS WET AND SMELLS LIKE MOLD. THE FACILITY WAS TRYING TO STEAM CLEAN IT

Building and Grounds. The childcare center shall be clean, safe, sanitary, and in good repair at all times to ensure the safety and well-being of children, employees, and visitors. This requirements is not met as evidence by LPAs observation of several electronic outlets being uncovered, backpack hooks observed to be broken with nails sticking out of the wall, chiped paint on walls, stained carpet, and a broken toilet in the school age bathroom. Physical Plant. LPAs observed an exposed water fountain not in working condition accessible to children and the short gate at the entrance unabke to close orrectly.

Buildings and Grounds — LPA observed the front gate to the entrance of the school is not closing properly and the locking mechanism is broken. This poses a potential Health and Safety risk to children in care.
