Drinking water lead testing — Based on interview and record review, the licensee did not comply with the section cited above which poses an immediate health, safety or personal rights risk to persons in care.

Health-Related Services For each prescription medication, the licensee shall obtain, in writing, approval and instructions from the child's authorized representative for the administration of the medication to the child. This documentation shall be kept in the child's record. The requirement is not met by LPA observed the facility does administer Incidental Medical Services, (IMS). However the facility does not have an IMS plan in place. 3 children do not have IMS documents in their file. This poses an immediate health risk to children in care.

Personal Rights — interviews conducted that state the administrator had verbal confrontations with a couple staff members in front of children and a staff member also yelled at children in care. This poses/posed a immediate Health and Safety risk to children in care.

Limitations on Capacity and Ambulatory Status — Interviews conducted, documents reviewed and received and LPAs own observations of preschool and school age children being commingled all together in the preschool middle class room. This poses an immediate Health and Safety risk to children in care.

Responsibility for Providing Care and Supervision — LPA Interviews conducted, video footage reviewed and documents received and reviewed, of a preschool child who wondered out of the facility without a staff member present. This poses/posed a immediate Health and Safety risk to children in care.

Indoor Activity Space for School-Age Children — Per interviews and documents reviewed, it was stated that preschool and school age children are commingled in the morning and in the afternoon due to staffing issues. This poses an immediate risk to the health, safety and personal rights of children in care.

Teacher Aide Qualifications and Duties — An aid and van driver, and who opens the day care has been left alone with Preschool and school age children and no other staff around in the morning and in the afternoon. This poses an immediate risk to the health, safety, and personal rights of child in care.

Criminal Record Clearance — LPA observed during file review and staff names taken, staff #4 not fingerprint cleared and staff #1 is not fingerprint associated to this facility. This poses an immediate risk to the health, safety, and personal rights of child in care. A civil penalty is being assessed.

Buildings and Grounds — Based on observation and interview the licensee did not comply with the section cited above which poses/posed a potential health, safety or personal rights risk to persons in care. LPAS OBSERVED A HOLE IN A WALL IN THE PRESCHOOL 2-3 YEAR OLD CLASS ROOM.

Food Service — Based on observation and interview, the licensee did not comply with the section cited above in which poses/posed a potential health, safety or personal rights risk to persons in care. LPAS OBSERVED FOOD IN ZIP LOCK BAGS IN THE FREEZER AND FOOD IN THE REFRIDGERATOR WITH OUT EXPERATION DATES.

Infant Care Center Fixtures, Furniture, Equipment and Supplies. Infant changing tables shall: Be maintained in good repair and safe condition. The requirement is not met as evidence by: LPA observed the changing pad in the 2-3 year old classroom having exposed foam showing. This poses a potential health and safety risk to the children in care.

Health-Related Services — LPA and Director observed several medications belonging to 3 children whom are no longer in care and no longer enrolled in the facility. This poses a potential health and safety risk to the children in care.

Personnel Requirements — ; On the departments background Check System, it shows Staff #2 is fingerprint cleared however, they are not associated to the facility. This poses a potential risk to the health, safety, and personal rights of children in care. A civil penalty is being assessed.

Personnel Requirements — Staff # 1/3/4/5 and 6 are missing Pediatric First Aid and CPR. The reason all these staff need this is due to, the position of the staff being van drivers or the opener or closer teacher who are left alone with children. This poses a potential risk to the health, safety, and personal rights of children in care.

Mandated reporter training — Based on file review Staff # 1/5 and 6 are missing the mandated reporter training certificate, which poses a potential health and safety risk to children in care.

Current roster of children — the facility not having a complete roster available. This poses a potential risk to the health and safety of children in care.

Personnel Records — no current copy of the Personnel Report (LIC 500). This poses a potential health and safety risk to the children in care.

Reporting Requirements — Per interviews it was stated that no one called or faxed in an incident report that occurred on 4/4/24 until 04/10/24. This poses a potential Health and Safety risk to children in care.

Buildings and Grounds — LPA observed the front gate to the entrance of the school is not closing properly and the locking mechanism is broken. This poses a potential Health and Safety risk to children in care.

Fixtures, Furniture, Equipment and Supplies — LPA observed on the steps of the climbing apparatus, the metal stepping bars are all exposed with most of the cushion material peeling off. This poses a potential Health and Safety risk to children in care.
