Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. In some areas of the outdoor play structure there is rust, rust that is peeling, and the paint has faded.

This was not met as evidenced by…. Based on LPAs observations there was wooden logs with splinters and left over plywood leaning against the fence and dry brush and debris in and around the playground area, easily accessible to children in care. This poses a potential health, safety and personal rights risk to children in care.
