LPA informed that Flu vaccine is required annually, but is optional, so long as the individual in question provides a written statement (signed and dated) opting out of Flu shot for given year which poses a potential health, safety or personal rights risk to persons in care.
Family home — clean, safe, heated and ventilated — Based on observation , the licensee did not comply with the section cited above. L. Cabrera observed dog feces in the living room where the children were watching television. LPA observed dog feces in the back yard, which poses a potential health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on observation and interview, the licensee did not comply with the section cited above. Facility is a two-story home and stairs were not fence or barricaded, which poses a potential health, safety or personal rights risk to persons in care.
