Responsibility for Providing Care and Supervision — One child was left in the facilitiy bathroom unattended & without supervision which poses an immediate risk to the health, safety, or personal rights to persons in care.

Disaster and Mass Casualty Plan — Based on interview and record review, the licensee did not comply with the section cited above in that the facility did not provide proof of a disaster drill conducted after 12/2025 which poses a potential health, safety or personal rights risk to persons in care.

Teacher-Child Ratio — Staff briefly operating out of ratio due to a staff shortage related to the sudden passing of a staff member, main director’s medical leave of absence, and initial interim director's leave of absence for travel.

Buildings and Grounds — Interviews disclosed that evidence of rodents were found inside the facility premises which poses a potential risk to the health and safety of the children in care.

Child's Records — Based on observation, and tour of the facility, the office where the staff and children's files are stored was left open on 9/15/25, and the drawers of the file cabinet where the files were stored were not secure which poses a potential health and safety risk to children in care.

Teacher Aide Qualifications and Duties — Based on record review, 1 out of 3 classrooms was operating with 2 teacher aides on 9/15/25. Per Executive Director, 2 qualified teachers were on leave that day which posed a potential health and safety risk to children in care.

Personnel Requirements — Based on observation and record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. During record review LPA observed that S1 and S2 did not have the LIC 503 in the staff file.

Outdoor Activity Space — , based on Interviews of reporting party and staff. Who have stated that they have observed staff tripping on wired mesh in the grass area. LPA also observed wire mesh by the grass area that is hazardous to children and staff.

Buildings and Grounds — , based on Interviews of reporting party and staff. Who have stated that they have observed a rat running in the playground.

The child care center shall be clean, safe, sanitary and in good repair at all times to ensure the safety and well-being of children, employees and visitors. Based on observation and interview. Staff and parents have stated there was construction that was going on during the hours of operation. LPA observed cones on the roof of the building indicating that work was being done on the roof. — requirement not met
