Personnel Records — Based on observation and record review, the licensee did not comply with the section cited above when two of three staff files did not contain the required documents, which poses/posed a potential health, safety or personal rights risk to persons in care.
Disaster and Mass Casualty Plan — Based on interview, and record review, the licensee did not comply with the section cited above when the disaster drills were not conducted at least every six months. Last drill was completed in January 2024, which poses/posed a potential health, safety or personal rights risk to persons in care.
Drinking water lead testing — Based on interview and record review, the licensee did not comply with the section cited above when the CCC did not complete the testing prior to their deadline of January 1, 2023, which poses/posed a potential health, safety or personal rights risk to persons in care.
