To receive safe, healthful and comfortable accommodations, furnishings... — Based on interviews conducted it was disclosed by S1-S3 and C1-C2 that S1 and S4 were involved in a verbal altercation in front of children. Buildings and Grounds — Based on observations the licensee did not comply ,with the section cited above . During the tour LPA observed black, brown , yellow stains and holes on the carpet . Water dispencer had stains and dust . LPA observed couch cushions soiled , which poses/posed a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on records review and interview , the licensee did not comply with the section cited above . LPA reviewed 5 satff files and observed 3 staff members were missing records of immunization , which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on the records review , the licensee did not comply with the section cited above. LPA reviewed 5 records and 2 out of 5 were missing Health Screening reports , which poses/posed a potential health, safety or personal rights risk to persons in care.
