Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above in that cleaning supplies (ajax, comet, lysol) were observed in an off limits office behind a baby gate and in a very high shelf in the children's bathroom, not behind a lock, which poses/posed a potential health, safety or personal rights risk to persons in care. No children were present in the areas and cleaning supplies/poisons were inaccessible, which is why a type B citation was issued instead of a type A.
Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in that S2 did not have a current mandated reporter certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.
Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in that 4 out of 4 staff present during the inspection did not have a current CPR/First Aid certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.
