Limitations on Capacity and Ambulatory Status — Based on observations, interviews conducted, and records reviewed, LPA observed five school-age children present outside with six preschoolers upon entry into the facility and all children were eating lunch outside together during the visit which poses an immediate safety risk to children in care.

Disaster and Mass Casualty Plan — Based on record review and Directors admission, the licensee did not comply with the section cited above in that a current disaster drill was not on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Child's Medical Assessments — Based on record review, the licensee did not comply with the section cited above in 1 out of 2 children did not have a medical assessment on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Immunizations — Based on record review, the licensee did not comply with the section cited above in 1 out of 2 children did not have their immunization's on file, which poses/posed a potential health, safety or personal rights risk to persons in care.

Disaster and Mass Casualty Plan — Based on record review and Licensee admission, the licensee did not comply with the section cited above in that a disaster drill has not be completed and no log was available for review, which poses/posed a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation, the licensee did not comply with the section cited above in that cleaning supplies (ajax, comet, lysol) were observed in an off limits office behind a baby gate and in a very high shelf in the children's bathroom, not behind a lock, which poses/posed a potential health, safety or personal rights risk to persons in care. No children were present in the areas and cleaning supplies/poisons were inaccessible, which is why a type B citation was issued instead of a type A.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in that S2 did not have a current mandated reporter certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in that 4 out of 4 staff present during the inspection did not have a current CPR/First Aid certificate, which poses/posed a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on observation, interviews conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure required immunizations were maintained for all personnel which poses a potential health risk to children in care.

Child's Medical Assessments — Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure Physician's Report (LIC 701) forms were completed and maintained for three out of four children which poses a potential health risk to children in care.

Personal Rights — Based on observation, interview conducted, and records reviewed, the licensee did not comply with the section cited above as licensee did not ensure LIC 613A forms were signed for two out of four children which poses a potential personal rights risk to children in care.
