Personal Rights — Based on manufactures information, it was determined that facility utilized the climbing structure outside of the manufacturer's instructions. As well as not treating the child healthful. This is an immediate risk to the health and safety of children in care.

Mandated reporter training — Based on observation, interview, record review, the licensee did not comply with the section cited above in 2 out of 6 staff did not have a current childcare mandated reporter certificate, which poses a potential health, safety or personal rights risk to persons in care.
Staff immunization requirements — Based on observation, interview, record review, the licensee did not comply with the section cited above in 1 out of 6 staff did not their immunization record which poses a potential health, safety or personal rights risk to persons in care.
Personal Rights — Based on observation and interview, facility did not sure that a child was accorded dignity, as camera footage showed S4 violating C1 personal rights in two different occassions. This is a potential risk to the health, safety, and personal rights of children in care.
Reporting Requirements — Based on the information obtained unuslal incident that happend on June 11, 2024 was not reported to the department in a timely manner which poses a potential risk tot the health and safety of the children in care.
