Staff-Infant Ratio — Based on observation and interview, Licensee did not ensure that the infant classroom is kept in ratio at all times. LPAs observed 5 infants with one staff during today's inspection. This poses an immediate risk to the safety of children in care.
Personal Rights — Based on observation and interview, Staff 5 did not ensure that Child 4 was removed from the infant rocker in an appropriate manner by tilting the rocker forward causing Child 4 to fall out of it. This is an immediate risk to the personal rights of children in care.

Fixtures, Furniture, Equipment and Supplies — Based on observation, the licensee did not comply with the section cited above in 2 of 3 classrooms had unscreened open-faced heaters with in reach which poses/posed a potential health, safety or personal rights risk to persons in care.
Indoor Activity Space for Infants — Based on observation the licensee did not comply with the section cited above in 3 of 3 children were in cribs inside the indoor activity space which poses/posed a potential health, safety or personal rights risk to persons in care.
Infant Care Center Sleeping Equipment — Based on observation, the licensee did not comply with the section cited above in 3 of 3 children used blankets inside of the crib which poses/posed a potential health, safety or personal rights risk to persons in care.

Teacher Aide Qualifications and Duties — Based on record review and interview, Licensee did not ensure that Staff 1 met the educational requirements prior to hiring them as an assistant in the facility. LPAs observed Staff 1 providing care in the toddler classroom. This poses a potential threat to the personal rights of children in care.

On 3/26/24, LPAs observed Child1 who is over 2 years old, in the infant classroom. This poses a potential safety risk to children in care.

(1) The sleeping area for infants shall be physically separate from the indoor activity space. — Based on observation and interview, the Licensee did not ensure that the infant napping space is physically separate from the indoor activity space. This poses a potential risk to the health, safety and personal rights of children in care.
Infant Care Center Sleeping Equipment — Based on interview and observation, Licensee did not ensure that Child 3 was provided appropriate napping equipment. Per Staff 5, Child 3 uses a big pillow in the classroom to nap. This is a potential risk to the health, safety and personal rights of children in care.
