Infant Care Center Sleeping Equipment (b) A crib or portable-crib meeting United States Consumer Product Safety Commission safety standards shall be provided for each infant who is unable to climb out of a crib Based on observation and interviews, the licensee did not comply with the section cited above in which there was not enough cribs for infants in care and observed 2 infants sleeping in pack n plays, which poses an immediate health safety or personal rights risk to persons in care.

Fixtures, Furniture, Equipment and Supplies — Based onobservation, the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care. Specifically, the infant classroom bathroom showed signes of mold which indicates it is not being cleaned regularly.
Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care. Specifically, LPA observed several tripping hazards, metal objects on the ground which could be a choking hazard, bent fencing with sharp edges, broken play equipment, and loose/broken gates.
Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in 3 out of 4 which poses/posed a potential health, safety or personal rights risk to persons in care. Specifically, S1-S3 did not have immunizations in their file.
Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in 3 out of 3 files which poses/posed a potential health, safety or personal rights risk to persons in care. Specifically, S1-S3 were missing their TB testing verficiation in their staff files.
