Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above in LPA's finding 8 pieces of glass on the play ground where ther children play which poses an immediate health, safety or personal rights risk to persons in care.

Temporary suspension of a license — Based on observation, interview, and record review, the licensee did not comply with the section cited above, by not providing a copy of the accusation summary to the parents which poses an immediate health, safety or personal rights risk to children in care.

The licensee shall require each recipient of the summary of the accusation to sign a statement indicating that he or she has received the document and the date it was received. This requirment was not met as evidence by: Based on observation, interview, and record review, the licensee did not comply with the section cited above, by not having all parents sign the LIC 9224 which poses an immediate health, safety or personal rights risk to children in care.

Personal Rights — Based on observation and record review, the licensee did not comply with the section cited above, S4 restraining child C1 in care which poses an immediate, health, safety or personal rights risk to children in care.

The surface of the outdoor activity space shall be maintained: Free of hazards including, but not limited to, holes, broken glass and other debris, and dry grasses that pose a fire hazard This requirment was not met as evidence by Based on observation and interview , the licensee did not comply with the section cited above by having glass on the play ground when children play which poses an potential health, safety or personal rights risk to children in care

Limitations on Capacity and Ambulatory Status — Based on observation, and interview, the licensee did not comply with the section cited above by allowing infant C1 to be on the playground with preschool children present which poses an potential health, safety or personal rights risk to children in care.

Napping Equipment — Based on interview, the licensee did not comply with the section cited above in, Staff did not ensure sheets were washed weekly which poses an potential health, safety or personal rights risk to children in care.

Limitations on Capacity and Ambulatory Status — Based on[observation, and interview, the licensee did not comply with the section cited above by allowing infants to be in the preschool room with preschool children present which poses an potential health, safety or personal rights risk to children in care.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in one count of the Director not having a current mandated reporter which poses/posed a potential health, safety or personal rights risk to persons in care.

Personnel Records — Based on record review, the licensee did not comply with the section cited above in 2 counts of S1 and S1 not having a tb test on file which poses/posed a potential health, safety or personal rights risk to persons in care.

Disaster and Mass Casualty Plan — Based on record review, the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care. Specifically, facility has not conducted a fire drill in the last 11 months.

Indoor Activity Space — Based on observation, the licensee did not comply with the section cited above, which poses/posed a potential health, safety or personal rights risk to persons in care. Specifically, floors and basebordes were in need of deep cleaning. Rugs were in need of deep cleaning or replacing.

Mandated reporter training — Based on record review, the licensee did not comply with the section cited above in 4 out of 5 staff files, which poses/posed a potential health, safety or personal rights risk to persons in care. Specifically, S2-S5 did not have mandated reporter training in their straff files.

Staff immunization requirements — Based on record review, the licensee did not comply with the section cited above in 2 out of 5 staff files, which poses/posed a potential health, safety or personal rights risk to persons in care. Specifically, 2 staff files did not have immunizations.

Personnel Requirements — Based on record review, the licensee did not comply with the section cited above in 4 out of 5 staff files, which poses/posed a potential health, safety or personal rights risk to persons in care. Specifically, 4 out of 5 staff files were missing the LIC 503.

Personnel Records — Based on record review, the licensee did not comply with the section cited above in 4 out of 5 staff files, which poses/posed a potential health, safety or personal rights risk to persons in care. Specifically, 4 out of 5 staff were missing their TB clearances.

Personnel Requirements — based on interviews, staff did not provide adequate supervision to children, which poses a potential health, safety, and personal rights risk to children in care.

Buildings and Grounds — based on interviews and observations, the facility is not clean and sanitary, which poses a potential health, safety, and personal rights risk to children in care.
