Responsibility for Providing Care and Supervision — No staff saw Child #1’s incident, in which child #1 sustained an injury requiring medical attention in which child sustained a 1 cm laceration to the upper lip involving the vermilion border, which required three sutures, which poses an immidtae risk to the health and safety of the children in care.

Responsibility for Providing Care and Supervision — LPA Perry determined that Child #1 was left unsupervized for approximately 34 minutes.

Responsibility for Providing Care and Supervision — LPA Perry determined that Child #1 being left in the classroom during a parent event on 02/13/2026 under the supervision of another parent, in which child #1 was confirmed to be signed in to the facility., which poses an immediate risk to the health and safety of the children in care.

Responsibility for Providing Care and Supervision — staff interviews. S1 left C1 in the classroom alone, unsupervised. C1 was found by another S2. This is an immediate health and safety risk to children in care.

Reporting Requirements — Based on interviews and record review, the licensee did not comply with the section cited above as the facility did not submit to the department the LIC624 written report within the required 7 days hich poses a potental risk to the health and safety of the children in care.

Health-Related Services — Based on observations, interviews and record review, the licensee did not comply with the section cited above in C#7 was observed to have expired medication which was observed on the prescription label which poses/posed a potential health, safety or personal rights risk to persons in care.

Reporting Requirements — Based on interviews and record review, the licensee did not comply with the section cited above as the facility did not report to the Department the Unususal Incident of child #1 exited through the emergency exit door with no supervision in the preschool yard, which poses/posed a potential health, safety or personal rights risk to persons in care.

Notice to parents that site visit reports are available — Based on record review, the licensee did not comply with the section cited above in Child #1 did not have LIC9224 on file which poses a potential health, safety or personal rights risk to persons in care.
