Criminal Record Clearance — Based on interiew and record review, the licensee did not comply with the section cited above in having S10 work at the facility and interact with children which posed an immediate health, safety or personal rights risk to persons in care.

Limitations on Capacity and Ambulatory Status — Based on observation and interview, the licensee did not comply with the section cited above C6 is 2 years of age and was moved to preschool classroom without approval which poses a potential health, safety or personal rights risk to persons in care.

Health-Related Services — Based on interiew and record review, the licensee did not comply with the section cited above in not notifying a parent of an incident involving a daycare child at the time of pickup which posed a potential health, safety or personal rights risk to persons in care.

Personal Rights — Based on interviews, the licensee did not comply with the section cited above in having staff handle children in a rough manner, which posed a potential health, safety or personal rights risk to persons in care.

Teacher-Child Ratio — Based on interviews, the licensee did not comply with the section cited above in ensuring staff are not supervising more than 12 children without additional staff present, which poses a potential health, safety or personal rights risk to persons in care.

Personal Rights — Based on interviews, the licensee did not comply with the section cited above in having staff speak aggressively towards children, which posed a potential health, safety or personal rights risk to persons in care.

Indoor Activity Space for School-Age Children — Based on interviews, the licensee did not comply with the section cited above in comingling children across age groups, which poses a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observations, record review, and interviews, the licensee did not comply with the section cited above not ensuring the facility is free of spider webs, insects, and mosquitoes, which posed a potential health, safety or personal rights risk to persons in care.

Food Service — Based on observations, record review, and interviews, the licensee did not comply with the section cited above by not consistently following the posted menu, running out of milk, and complaints of spoiled food and milk which posed a potential health, safety or personal rights risk to persons in care.

Outdoor Activity Space — Based on observation, the licensee did not comply with the section cited above in LPA witnessed play sets with spider webbs in multiple areas, which poses/posed a potential health, safety or personal rights risk to persons in care.

Health-Related Services — Based on observation, interview, and record review, the licensee did not comply with the section cited above there is no written plan avaiable for Incidental Medical Services (IMS) for 3 children, C4, C6, and C8, which poses a potential health, safety or personal rights risk to persons in care.

Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above in outside shed was unlocked with chemicals, old paint cans, that could cause harm to children in care if touched, This poses a potential health, safety or personal rights risk to persons in care.

Staff immunization requirements — Based on record review, two staff were missing documentation of required immunizations, which poses a potential health, safety and/or personal rights violation to persons in care.

(g)..(1) Except as specified in (3) below, good physical health shall be verified by a health screening, including a test for tuberculosis, performed by or under the supervision of a physician not more than one year prior to or seven days after employment or licensure.
