Family home — home free of safety hazards — Based on observation fire extinguisher had a service tag dated Aug 2023, picture taken, the licensee did not comply with the section cited above in [1] out of [1] fire ext. that does not meet State Fire Marshall which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on observation LPA Calderon observed child #4 with an attached pacifier during nap time sleeping in the play pen, the licensee did not comply with the section cited above in [1] out of [1] person which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on LPA Calderon's observation during staff record review, Staff #2 did not have mandated reporter training on file, the licensee did not comply with the section cited above in [1] out of [1] person which poses a potential health, safety or personal rights risk to persons in care.
Infant Safe Sleep — Based on LPA Calderon observation during record review did not observe Child #4 LIC9227, Indiv. Infant Sleep Plan, Per Licensee interview stated will have for completed and placed in child's records, the licensee did not comply with the section cited above in [1] out of [1] person which poses a potential health, safety or personal rights risk to persons in care.
