Criminal Record Clearance — Based on interview and record review, and observation, during today’s inspection, LPA observed an adult providing care and supervision to children in care that did not request a transfer of a criminal record clearance prior to working in the home which poses a potential health, safety or personal rights risk to persons in care.
Family home — children supervised at all times — During today’s inspection, the Licensee informed LPA Garcia that co-licensee Jeffrey Brooks is absent from the home more than 20% of the time, demonstrating non-compliance with regulatory requirements which poses a potential health, safety or personal rights risk to persons in care.
Family home — in-ground pool drowning prevention requirements — Based on observation, the licensee did not comply with the section cited above. Licensee has not obtained a life ring that is approved by the United States Coast Guard and has not been keeping a log for her daily inspections of the drowning prevention features which poses a potential health, safety or personal rights risk to persons in care.
Mandated reporter training — Based on interview and record review, the licensee did not comply with the section cited above. Licensee's Mandated Reporter Training was completed on 10/16/2023 and was not renewed by 10/16/2025. Staff #1 (S1) has not completed the required traininig which poses a potential health, safety or personal rights risk to persons in care.
Family home — preventive health training, including pediatric CPR and first aid — Based on record review, licensee failed to maintain a current CPR/FIRST AIDE certificate, which poses a potential health, safety or personal rights risk to persons in care.
