Family home — clean, safe, heated and ventilated — Based on observation, the licensee did not comply with the section cited above in In the outdoor area, LPA's observed one cracked play structure in at least two places. A tall tree which has thorns exposed at the bottom. Dog poop in areas where children play. One tin bin with no locks/latches; this bin had one old suitcase, one tool box and BBQ tongs. The left side yard has a free standing board up, not attached to anything. There is storage of multiple items that should not be accessible to children. This poses an immediate health, safety or personal rights risk to persons in care.
Family home — home free of safety hazards — Based on observation, the licensee did not comply with the section cited above. Medicine cabinet does not have lock or latch on it, kitchen cabinets had items that were unsafe for children; multiple lighters, tools and sharps like knives, cheese grater, skewers, carving fork, and wine bottle opener. Although there was a latch under the kitchen sink, there were items which are considered poisons and should be key-locked. There was also a shaver which was left in the on-limits restroom. This poses an immediate health, safety or personal rights risk to persons in care.
Staff immunization — influenza, pertussis and measles — Based on record review, the licensee did not comply with the section cited above in 1 out of 3 persons, which poses/posed a potential health, safety or personal rights risk to persons in care.

Mandated reporter training — (1) On or before March 30, 2018, a person who, on January 1, 2018, is a licensed child day care provider, administrator, or employee of a licensed child day care facility shall complete the mandated reporter training provided pursuant to paragraphs (2) and (3) of subdivision (a), and shall complete renewal mandated reporter training every two years following the date on which he or she completed the initial mandated reporter training. Based on record review, the licensee did not comply with the section cited above in 3 out of 3 staff, which poses/posed a potential health, safety or personal rights risk to persons in care.
