Buildings and Grounds — Based on observation and interview, the licensee did not comply with the section cited above. A bottle of carpet spot removal chemical was observed under the children's hand washing sink in classroom #4 and was accessible to the children, which poses an immediate health, safety or personal rights risk to persons in care.

Teacher-Child Ratio — Based on observation and interview per LPA's observation, it was observed that there was 1 teacher with 17 children. S1 was out of ratio for about 3 minutes. This poses an immediate safety risk for the children in care.

Supervision shall include visual observation. This requirement was not met: Based on interviews conducted C1 and C2 were not being supervised by S1 for 1 to 3 minutes, children were in the next classroom alone. This poses an immediate risk to the safety and health of the children in care.

Fixtures, Furniture, Equipment and Supplies — Based on observation, the licensee did not comply with the section cited above. The boys' restroom door fell off the hinge when staff opened the door during today's visit, which posed a potential health, safety or personal rights risk to persons in care.
Food Service — Based on observation, and interview, the licensee did not comply with the section cited above. LPA observed 4 reusable snack plastic containers, that were being used to store differnt/left over snacks, that were removed from the original bags, the 4 containers did not have date of the current snacks that were in the containers, which poses a potential health, safety or personal rights risk to persons in care.
Disaster and Mass Casualty Plan — Based on interview and record review, the licensee did not comply with the section cited above. Director confirmed the facility has not conducted disaster drill and did not keep documentation of the drill, which posed a potential health, safety or personal rights risk to persons in care.
101238 Buildings and Grounds (a) The child care center shall be clean, safe, sanitary and in good repair at all times to ensure... (1) The licensee shall take measures to keep the center free of flies, other insects, and rodents. This requirement was not met: Based on interviews conducted S4 observed cockroaches inside the classroom about a month ago. This poses a potential risk to the safety and health of the children in care.
Teacher-Child Ratio — Based on records reviewed, facility did not have a substitute list for backup staff in case of an emergency in which, the facility did not have a preschool opener on 3/12/24, 3/13/24, and 3/14/24. This poses a potential health and safety risk to the children in care.
